Create a Feedback Intake Form in Fibery
Create a Form View for the database that should receive submissions, choose its visible fields, and use Preview before sharing its link.
Before you start: Have a Feedback database and permission to create Views. Keep internal priority and customer-sensitive fields out of the public form.
Worked example: Collect a request title, problem description and contact email for an onboarding study. A submitted request should create one Feedback record, without exposing an internal priority field.
Step-by-step guide
Choose a dedicated destination
Use a Feedback database with only the fields you need to collect. Separate incoming requests from internal delivery decisions.
Create the Form
Click + beside the Space, select Form, and choose Feedback in the Database dropdown at the top right.
Configure the questions
Choose the fields shown to submitters. Give the description a concrete prompt: What prevented you from inviting a teammate?
Submit through Preview
Use Preview with clearly labeled sample data. It creates a real database record, so locate and remove the sample after checking the result.
Share after an access check
Use Share to generate the form link. Check the intended external submission flow and the Space/database arrangement before distributing it.
Common mistakes
Treating Preview as a sandbox
Preview submissions enter the connected database and may trigger automations. Use a recognizable test record.
Exposing internal relations
Review choices visible in relation selectors. A public submission flow should not reveal an unrelated customer list.
