Patient Safety Impact
If any change affects clinical workflows, patient data handling, or clinical decision support, lead with the safety impact assessment. Document what changed, how it affects clinical workflows, and what clinicians need to know before encountering the change. Patient safety information must be prominent, not buried.
HIPAA and Compliance Updates
Document any changes affecting PHI handling, access controls, audit logging, or data encryption. Include the compliance rationale and any updated BAA (Business Associate Agreement) terms. IT administrators need this information for their compliance documentation.
Clinical Workflow Changes
For any UI or workflow change: show before-and-after screenshots, explain the new workflow step by step, and provide training resources. Clinical users learn workflows deeply and changes cause friction. Include a "why this changed" section so users understand the improvement, not just the disruption.
EHR Integration Updates
Document changes to HL7 FHIR endpoints, CDA document handling, or EHR system integrations. Include migration guides for any API changes. Healthcare integrations are complex and changes require coordination with hospital IT teams.
Downtime and Maintenance
Healthcare systems require advance notice for any downtime: minimum 2 weeks for scheduled maintenance, immediate notification for emergency maintenance. Include fallback procedures for clinical users during downtime.
