HIPAA Impact Assessment
During backlog grooming, assess each story for HIPAA impact. Stories that create, access, or modify Protected Health Information (PHI) need additional security review time. Tag stories with "PHI-Access" label and route them through the security review workflow.
Clinical Validation
Stories that affect clinical workflows require validation with clinical stakeholders (physicians, nurses, pharmacists). Schedule clinical review sessions within the sprint for these stories. Clinical validation cannot be skipped or deferred.
Sprint Goal with Patient Safety
If stories affect patient-facing features, the sprint goal should reference patient safety. Example: "Medication interaction alerts display within 2 seconds with zero false negatives — clinical validation complete by sprint end."
Estimation with Compliance Overhead
Add compliance overhead to estimates: stories accessing PHI get an additional 2-3 points for audit logging, access controls, and HIPAA documentation. Stories affecting clinical workflows add 2 points for clinical validation sessions.
Definition of Done (Healthcare)
Extend the DoD: HIPAA-compliant data handling verified, audit trail captures all PHI access, clinical workflow validated with clinicians, accessibility tested (Section 508), and documentation updated for clinical users.
