Create Product Intake Forms in Notion
Add a Form view to a request database so submissions become structured records. Form visibility and database visibility are separate choices and must both be checked.
Step-by-step guide
Check access and scope
Full access is required to create a form from an existing database. Use a test database first if the request fields contain sensitive information.
Create request properties
Create request properties for problem, requester, evidence and desired timing.
Add a Form view
Add a database view and select Form. Use + beside the database views or Add view, then select Form; creating a form from an existing database requires Full access.
- Worked example: Ask What are you trying to achieve? before asking Which feature do you want? An invitation example should capture the current workaround and a source link rather than forcing a solution.
Write useful questions
Edit question labels and require only the information needed for triage. Question names can synchronize with database property names; switch off that synchronization when the internal label must stay stable.
Preview sharing
Preview the form and check its sharing audience.
Submit a sample request
Submit a sample request and assign a reviewer in the resulting database row.
Verify the result
A sample submission creates exactly one readable request record and intended respondents can access the form.
Common mistakes
Avoid the main failure mode
Renaming a synced question can rename its database property. Turn off name synchronization when labels need to differ.
Check the handoff edge case
Making every intake question required can force low-quality guesses. Require only information genuinely necessary for triage.
