Incident Overview

Start with the facts: incident date and time, duration, severity level (P0-P3), number of users affected, and financial impact if measurable. Include a one-sentence summary that captures the essence: "API gateway certificate expired, causing 100% API failure for 47 minutes."

Timeline

Reconstruct the incident minute by minute. Use Notion's timeline or table format: Timestamp, Event, Who Noticed, Action Taken. Start from the first sign of trouble through detection, response, mitigation, and resolution. Include timestamps in UTC for cross-timezone clarity.

Root Cause Analysis

Use the "5 Whys" technique: ask "why?" five times to move from symptoms to root causes. Document each level. The root cause is rarely "someone made a mistake." It is usually a process, tool, or system gap that allowed the mistake to have an impact.

Impact Assessment

Document the full impact: users affected (count and percentage), revenue impact, SLA implications, customer support tickets generated, and brand/trust impact. Quantify wherever possible. This data justifies investment in prevention.

Action Items

Every postmortem must produce action items with owners and deadlines. Categorize: Immediate (this sprint), Short-term (this quarter), Long-term (backlog). Each item should prevent either this specific incident or this category of incident from recurring. Track action item completion in the Notion database.
